I have seen them both ways and can't really see why it matters other than for ease of torquing. They can rust into place so with the front end safely supported, remove the tire and wheel and soak the whole area with Kroil or something similar. Loosen the nut to where it is flush with the end of the bolt and tap to loosen the bolt in the D shape hole in the cam. Then you can try prying the cam a bit to loosen it from the two location ears. Do that on both sides and the cams may be a bit easier to turn for adjustment. If you are installing a one ton front end, install the adjusters with the lobe on the rear cam on the upper control arm pointing away from the frame. That will give max caster by pushing that end of the a arm as close to the frame as possible. Set the cam lobe on the front of the upper a arm to a neutral position to center the bolt in the slot. That will be about where zero camber will be achieved. Snug but do not torque the nut and do your measurements.
Once you have the caster set at maximum and even on both sides, and the camber at or close to zero on both sides, then torque the nut which will jam the cam against the ears and nothing will turn from there.

The parts book shows them from outside with nuts on inside, (SORRY). However the manual shows them from inside out, I have 2 coaches here and both are inside out. That would be bout the only way to get a torque wrench on the nuts.
