I finished up the manifold installation today between soccer games. I
ended up putting it on without the center bolt as several have suggested.
It sealed beautifully with Remflex gaskets. New bolts all around
including spring bolts and new donuts. Generous anti-seize so the next
time should be easy.

While reassembling, I think I may have a theory on why the center bolt is
there. It is just for faster assembly. The center bolt hole fits the
bolt shoulder exactly. The other four are over sized. If you start with
the center bolt, the whole assembly is centered and balanced making the
other four easy to start. Without the center bolt, you have to shift it
around awkwardly. From this point forward, we should officially call the
center bolt "the optional manifold assembly bolt".
