Years ago a worker fell into a charging car of a local steel mill blast furnace. The operation of the blast furnace was run by an IBM 1800 process control computer. Even though another worker saw what happened, there was no way for him to immediately shut down the car computer controlled operation. The computer sent the car to the top of the furnace where it dumped it's load of material, including the worker, into the top of the furnace. The operation was changed after that and big emergency stop buttons were installed at various locations where workers could to signal the computer to immediately cease all movements.
