I am installing a new gasket on my passenger side exhaust manifold. The old
one has no material between the second and third openings, and the new one
does. Should I remove this "extra" material?

Sir: it depends on which brand gasket you are using. If you have Remflex do not cut out, swap for correct ones. The header gaskets have different size ports than the stock manifolds. If you cut the centers out the hole is awful close to the bolt hole.

If your running a header, be sure to tighten the 4 bolt tht are in line
very tight and leave the top center one snug only.
Also after 50 miles, re tighten the 4 inline bolts
We have found that this procedure helps our customers from needing to
replace the gasket for 20,000 miles or longer.
