I did this several years ago. When I think about it more I believe that there was a steel plate with two bolts welded or brazed to it.
When the bolts broke the plate fell inside.
I was able to retrieve the plate with a magnet on a string. Then I drilled out the broken bolts and put in stainless steel 1/4" bolts and epoxied the heads against the steel plate.
Then I was able to reach into the space to get the plate with the two bolts into the holes so that someone could grab and hold the bolts on the outside.
I don't remember if I was able to reach in with my arm and hand or whether I used a tool. I may have clamped the head of a bolt into a vise grip or a channel lock pliers to give me the reach to maneuver the bolts through the holes. Whatever I did was successful.
