Thanks to carelessness shutting the door backing up into the garage, door
swung open and hinges got bent. Local body shop here in rural NC has given
up trying to get the door to shut tightly. So I am looking for an upper
hinge and better both hinges that are affordable. This may be a hopeless
quest.

Try a Chevy dealer. The hinge was the same as hinges used on older Surburbans which had two hinged doors on the rear. Probably late 70’s or early 80’s. You might find some in junkyards.
