I had some openings in the floor that I needed to fill in. This hole was 3 inches in diameter. My GMC floor is 3/4 inch thick. I made a square patch out of 1/2 inch plywood. Then dubbed off the corners due to my router would make a round corner. Traced the patch shape out on the floor. Set the router for a depth of 1/2 inch to match the patch thickness. After making the cut. I applied some good wood glue. Then stood oh the patch to make a good contact. No screws needed. I also had two 3/4 holes side by side. I simply made one rectangle patch to cover both holes at once. I have photo's if interested.

Put a drill bit stop on a 3/8" bit so the bit won't fully penetrate whatever thickness you're working with. You want maximum penetration without breaking through). Drill wells on a close, swiss cheese pattern, about 1" centers. Mop on West system epoxy, allowing the wells to completely fill. Works great. I recently checked a 12yr-old installation of the floor under carpet at an unprotected, windward sliding door known to have leaked many time during nor'easters/gales/hurricanes, plywood looked like the day I did the work.
