I bought my first replacement at Kragen Auto Parts in 1984. That became Checkers and now is O'Rielly's.
I have now put in 5 blower motors over the years. I keep getting free replacements under the lifetime warranty on the first on that I bought. I just got another from O'Riellys last year.
They used to come with the squirrel cage but now seem to only come with the motor.

My guess is insufficient oil at assembly or insufficient porosity in the bearing material to hold "spare" oil over time. I've revived a few GM blowers with a couple drops from the long reach spout oiler of "turbine oil" IF you catch it right when the noise starts and before the bearing is too sloppy. When I install a new one I always add a few drops at each end because I can't resist doing so with hope I won't have to do the job again.
