Are they safe ? I have carbon monoxide detectors. Do I need to leave a window cracked? It do cool down up here in the hills.

Reasonably safe, but not enough so they are legal everywhere. They are not approved and no new coach manufacturer I know of supplies them as standard equipment, most likely for liability reasons.
They also put a lot of water vapor into the air, and your windows will fog up a lot, even with some ventilation. Some people swear by them, others curse them. I know Dan Gregg likes his, but others do not.
I would not have one in my RV's but then I had a cousin and his wife and daughter killed in a carbon monoxide furnace tragedy (not RV related) and so I am super sensitive to the issue.

I had one at one time, but ony used it once.
It stunk up the area quite badly and if it can be
smelled, I suspect it ain't really good for you!!
I never used it again and it's not around any more.
I guess I must have pitched it into the trash!

I've had a Olympic Wave in my Cortez for many years. It's not enough in really cold weather, but normally once the RV is warm, it will keep it comfortable. NO NOISE. I've never noticed any particular odor. I leave a vent open a crack, and like others, I don't sleep with it on. I'd be tempted to add one to my GMC if I could find a good place to mount it.

Fear not. Read Steve F's post too.
I use a small portable Mr Heater. Mostly it is used in Quartzsite to help keep the heater fan from draining the batteries. Our coach has a lot of air leaks so we never had moisture on the windows in AZ. Did get some condensation in Canada but that was in high humidity and we even got it in the toad.
I stayed up all night a couple of times and never got a headache or other symptoms. I used it to supplement our regular heater in Canada - Lots of snow that late June night.
I put a T behind the stove and a quick disconnect next to the regular heater. We stow it in the closet. The manual says it has a low O2 cut toff. It uses a lot less propane than the heater. If I need fast heat in the coach, I turn on three burners on the stove. That produces a lot of moisture. We sealed a lot of leaks but never needed to crack a window due to the ones we could not seal. Air leaks are not bad; ours include the vent in the bath, the heater/AC ducts, the refrigerator venting which allows air into the bath, above the fridge, and around the paneling. We also have venting near the wheel wells, from the fan vent above the stove, around the door, and from around the windows. I bought my heater from from Dan Greg; since it did not kill him I figured it was safe. I do leave it on low at night but I am up every couple of hours. Never have had an objectionable smell and our CO sensor never went off; it is often set off by stove
top cooking to put that in perspective.
Doug Norton We are headed to Winnipeg sans coach. Lots of water here in North Dakota; it would have been a good year to plant rice. The fields look like the rice fields in the California delta. Poor farmers! Evacuations include persons who thought green areas near river beds were a good place to build.

I only use the forced air heater in the GMC when plugged in. That blower is a battery eater and can give you a dead battery overnight.
I had experimented with both Cat heaters and blue flame heaters.
My experience with the Cat heater is it uses no power bud does not seem to put out a lot of heat. Let me change that thought, the heat a Cat heater puts out is in the infra red area, a lot of the heat is felt where the Infra Red is directed, or objects get heated, air stays cool, except around the heater of course.
I have also ran a blue flame 30,000k btu heater in the GMC. Again, it uses no power and you do have to crack a window and a roof vent open.
the 30,000 btu is, in my experience, over kill and phisically too large but, it has NO PROBLEM heating the space in the GMC.
The Blue flame heater also has low oxy shut off and a thermostate for temperature control. Not all blu flame heaters have the thermostat so if you need it, make sure it has one.
If you chose the 20,000k btu blu flame heater, the phisical size will be smaller, which is good for us campers.
I know people that turn the stove on and that is the same as using a blu flame heater, only the heater consumes more gas.
My GMC does not have room to mount a blu flame heater on a wall or cabinet so I am envisioning being able to mount it temp and run the hose outside through a port and connect to a quick disconnect under the coach. I am nervous about trusting the seal of a propane quick connect inside the coach. Leaky propane inside the coach --- not so good.
Personally, I think the blu flame heater may be a better choice than a Cat heater but it is always the choice of the buyer.

I have a Cat heater, but seldom use it. Where are you guys mounting them ?
I've seen one mounted below the vacuum cabinet with a heat shield on the
back of the dinette, but that was the only one I've seen and I have an
embedded electric heater there so don't have room for both.

I have had an Olympian Wave 8 in my 1976 Royale for over 3 years. I live in Southern BC, Canada and it has provided excellent warmth through winter.
I replaced the Suburban Heater. The CAT is located on the wall on the left side of the entry door.
A tailored piece of Silver Insulation is affixed to the back of the dinette seat and provides adequate protection.
