Most early gm steering boxes have a play adjustment in them but I cannot speak for the gmc as haven't had that close of a look at mine yet. Typically there is a nut with a screw in the center of it. The nut locks the screw in place. Loosen the nut and turn the screw inward until you feel pressure and re check play, repeat. I do agree with the others who state you should be sure that the box is the source of the play. I don't see where you are located but up here in seattle there is a company called Redhead steering who sells reman gear boxes and have used them several times for truck gear boxes with twisted output shafts. The charge about $350 for a rebuilt box. Never had one come back.

When I turn the wheel the shaft going into the steering box turns but the output shaft attached to what I am calling the pitman arm does not turn at all. It only turns when I turn the wheel beyond the loose spot which is about 2 inches either way. Can the box be adjusted or is it just shot?

These gears very rarely wear out although the seals may leak at times. When you check free ply, make sure the engine is running to get a true indication. I think you'll be surprised at the difference in readings between engine on or off. They can be adjusted but go by the book to do it right.
